Please indicate specific addresses. Our Operations Department will handle the selection of suitable airports and the Medical Department will organize necessary ground transportation. At this stage, please indicate if you require only air transport, or the comprehensive option, that is “bed to bed”.
Please indicate approximate preferred or possible date of transport. The condition necessary for transport is the absence of contraindications, confirmation of which is the medical form, filled out and signed by the attending physician download HERE

Our Sales Department, together with the Department of Operations, will perform the initial estimation. The initial estimation will be sent to you up to 45 minutes from the receipt of inquiry and submission of basic data. The price indicated in the offer includes all costs.
Having established the above details, accepting the conditions of transport, finding no formal contraindications to the realization of transport and sending us a signed order, together with confirmation of payment for transport, the COMPLETION STAGE is initiated.
During the realization phase, the Customer is informed on a regular basis (in real time), on the progress of transport, i.e. all the key moments during its realization. What’s more, you will be assigned an employee for constant contact on the side of Smart Jet Medical Transport; this employee will have up-to-date knowledge of the status / progress of transport, and, if necessary, will offer any necessary clarifications.
